two new initiatives that will help you seek your personal

New ACO improves care, costs
The new Methodist Patient-Centered Accountable Care
Organization (MPCACO) can help achieve outstanding team
care. It’s the only new ACO program in the Dallas–Fort Worth
area, one of only six in Texas, and one of only 89 nationwide
participating in a Medicare coverage program coordinated by
the Centers for Medicare & Medicaid Services (CMS).
Doctors, nurses, hospitals, and other care providers come
together in ACOs to share responsibility for keeping patients
healthy. They work together to coordinate care for those on
Medicare to measurably improve the total cost, quality, and
satisfaction in health care for those patients.
“Methodist’s Patient-Centered ACO is committed to
transforming health care locally,” says Melissa Gerdes, MD,
FAAFP, assistant vice president and chief medical officer of
outpatient services and ACO strategy for Methodist. “We’re
excited to be part of the journey with CMS as we work together
to improve the health of the Medicare patients we serve.”
Care extends to the home
Methodist is extending quality care into the home health
environment through Methodist HomeCare in Dallas. Home
health services help those living with chronic diseases, such as
diabetes, heart failure, and COPD, as well as those recovering
from illness or injury.
Physician-directed services are offered in the convenience
and comfort of the patient’s home from skilled nurses and
therapists. The Philips Lifeline emergency medical alert is
also part of the plan of care at no extra cost.
